Self-referral is all you need to make an appointment…. however, reports and referrals from other professionals can optimise appointment outcomes.

 

You don’t need a referral to make an appointment, but you may want to see your GP or paediatrician to see if you are eligible for a Medicare rebate for Occupational Therapy services.

• Travel time may incur additional cost

• School/day-care visits can only be booked once written consent from the school or day-care is provided

NDIS Participant Bookings

• Fees are charged according the current NDIS price guide.  Payments are processed by Icon via the NDIS portal for agency managed participants or invoices are forwarded according to your plan management arrangement.

 

 

Private and Self-Funded Bookings

• Additionally, Icon provides specialised therapy support on a fee for service basis.  Please contact your private health insurance fund to clarify your cover and availability of rebate.

 

To reduce costs a written report is not included as standard as not all families require one. If you require a report, additional fees may apply.
